Blue card day, fall sports physicals

July 20, 2017—Any student in grades 7-12 interested in playing a fall sport must receive clearance from the school nurse PRIOR to tryouts. Clearance cards (blue cards) can be distributed to students on Tuesday, Aug. 1 from 9 a.m. to 12 noon in the nurse's office at Cohoes High School.

This is for student athletes with proof of a physical within the last 12 months from your doctor. Students should also bring a copy of the concussion information sheet signed by a parent.

Athletes in need of a physical

Students who have not received clearance from their doctor within the last year can come into the high school nurse’s office on Wednesday, Aug. 2 from 9-11:30 a.m. for a physical, or come to the Cohoes Middle School nurse’s office on Thursday, August 17, between 12 noon and 2 p.m. for a physical with the district’s physician, Dr. DiTursi from Harmony Mills Pediatrics.

The above dates are the ONLY days students will be able to receive a physical free of charge prior to the start of the season. Parents are not required to attend, but must send their child with a completed parent permission and health history form.

A special note for 7th graders and first-time athletes

The parent permission and health history form (distributed in the June report cards and available on the website) must be returned with physicals.
